Systematic review and meta-analysis of optimal P2Yââ blockade in dual antiplatelet therapy for patients with diabetes with acute coronary syndrome
Background: Patients with diabetes are at increased risk of acute coronary syndromes (ACS) and their mortality and morbidity outcomes are significantly worse following ACS events, independent of other comorbidities. This systematic review sought to establish the optimum management strategy with focus on P2Yââ blockade in patients with diabetes with ACS. Methods: MEDLINE (1946 to present) and EMBASE (1974 to present) databases, abstracts from major cardiology conferences and previously published systematic reviews were searched to June 2014. Relevant randomised control trials with clinical outcomes for P2Yââ inhibitors in adult patients with diabetes with ACS were scrutinised independently by 2 authors with applicable data was extracted for primary composite end point of cardiovascular death, myocardial infarction (MI) and stroke; enabling calculation of relative risks with 95% CI with subsequent direct and indirect comparison. Results: Four studies studied clopidogrel in patients with diabetes, with two (3122 patients) having primary outcome data showing superiority of clopidogrel against placebo with RR0.84 (95% CI 0.72â0.99). Irrespective of management strategy, the newer agents prasugrel (2 studies) and ticagrelor (1 study) had a lower primary event rate compared with clopidogrel; RR 0.80 (95% CI 0.66 to 0.97) and RR 0.89 (95% CI 0.77 to 1.02), respectively. When ticagrelor was indirectly compared with prasugrel, there was a trend to an improved primary outcome with prasugrel (RR 1.11 (95% CI 0.94 to 1.31)) particularly in those managed with percutaneous coronary intervention (PCI) (RR 1.23 (95% CI 0.95 to 1.59)). Prasugrel demonstrated a statistical superiority with prevention of further MI with RR 1.48 (95% CI 1.11 to 1.97). This was not at the expense of increased major thrombolysis in MI (TIMI) bleeding rates RR 0.94 (95% CI 0.59 to 1.51). Conclusions: This meta-analysis shows the addition of a P2Yââ inhibitor is superior to placebo, with a trend favouring the use of prasugrel in patients with diabetes with ACS, particularly those undergoing PCI

Evaluation Report of Prosperoâs Island: an Immersive Approach to Literacy at Key Stage 3.
Prospero's Island is an immersive theatre project created by Punchdrunk Enrichment and sponsored by Learning Partner, London Borough of Hackney (Hackney Learning Trust). The project sought to inspire and motivate studentsâ engagement with the English curriculum, and to develop an immersive approach to teaching literacy that would improve studentsâ learning.
Prosperoâs Island took place in a secondary academy in Hackney, London over two school terms (autumn 2014-spring 2015). The project was embedded in existing schemes of work, and included the following elements:
âą An immersive theatre installation for Year 7-8 students (aged 11-13 years); this took the form of an interactive game based on The Tempest; over a two-week period groups of students participated in this experience for a morning or afternoon (autumn term);
âą A Teaching and Learning Day (TALD) and eight twilight CPD sessions on immersive learning techniques for school staff and teachers across London (autumn term);
âą A return to the installation for one lesson, led by English teachers (autumn term);
âą Follow-on work by teachers to develop immersive learning in English lessons (spring term);
âą An independent evaluation of the project (autumn and spring terms)
Test and evaluation of load converter topologies used in the Space Station Freedom Power Management and distribution DC test bed
Power components hardware in support of the Space Station Freedom dc Electrical Power System were tested. One type of breadboard hardware tested is the dc Load Converter Unit, which constitutes the power interface between the electric power system and the actual load. These units are dc to dc converters that provide the final system regulation before power is delivered to the load. Three load converters were tested: a series resonant converter, a series inductor switchmode converter, and a switching full-bridge forward converter. The topology, operation principles, and tests results are described, in general. A comparative analysis of the three units is given with respect to efficiency, regulation, short circuit behavior (protection), and transient characteristics
Gemeinsam zum Innovationserfolg? Verbreitung und Effekte von Innovationskooperationen bei nicht forschungsintensiven Betrieben im Verarbeitenden Gewerbe
FĂŒr die BewĂ€ltigung der aktuellen Wirtschaftskrise wird Innovationen eine wichtige Bedeutung beigemessen. Nach dem Motto Stillstand ist RĂŒckstand ermöglichen Innovationen bei Unternehmen, gestĂ€rkt aus der Krise hervorzugehen und durch marktreife Neuentwicklungen frĂŒhzeitig von einem Wiederaufschwung zu profitieren. Produkt-, Service-, organisatorische oder technische Prozessinnovationen sind wichtige Stellhebel fĂŒr den langfristigen Unternehmenserfolg. Gerade die Entwicklung neuer Produkte kann durch erhöhte Investitionen in betriebliche Forschung und Entwicklung (FuE) gefördert werden. Viele Unternehmen in Deutschland weisen jedoch keine oder nur geringe FuE-AktivitĂ€ten auf und können daher von dieser innovationstrategischen Option in der Krise aufgrund mangelnder Ressourcen nicht in gleichem MaĂe Gebrauch machen wie stĂ€rker forschungsintensive Unternehmen. Die Teilnahme an FuE- bzw. Innovationskooperationen stellt daher insbesondere fĂŒr nicht forschungsintensive Unternehmen eine Möglichkeit dar, eigene fehlende FuE-Kompetenzen zu kompensieren und Innovationsprojekte zu realisieren, die aus eigener Kraft vielleicht nicht gestemmt werden könnten. Die Ergebnisse zeigen, dass nicht forschungsintensive Betriebe, die in FuE-Kooperationen neue Technologieimpulse aufnehmen, deutlich hĂ€ufiger Produktinnovationen hervorbringen und in vermarktbare Produkte umsetzen. --
FlexibilitÀt durch Technologieeinsatz? Nutzung und Erfolgswirkung flexibilitÀtsfördernder Technologien
FlexibilitĂ€t wird im globalen Wettbewerb immer wichtiger. Viele Kunden erwarten kurze Lieferzeiten bei hoher Termintreue und die FĂ€higkeit, Produkte kurzfristig an ihre individuellen WĂŒnsche anpassen zu können. In Industrieunternehmen wird daher neben entsprechenden organisatorischen Lösungen der Einsatz flexibilitĂ€tsfördernder Technologien als eine wesentliche Voraussetzung fĂŒr eine wandlungsfĂ€hige Produktion wichtiger. Der Einfluss der untersuchten Technologien auf FlexibilitĂ€tszielgröĂen wie Liefertreue und KapazitĂ€tsauslastung ist uneinheitlich. So wirken sich z. B. Supply Chain Management- und Industrieroboter/Handhabungssysteme positiv auf die Liefertreue aus, wĂ€hrend Produktionsplanungs- und -steuerungssysteme den Grad der KapazitĂ€tsauslastung positiv beeinflussen können. Computer Aided Manufacturing scheint dagegen keinen Einfluss auf diese GröĂen zu haben. Trotz dieser FlexibilitĂ€tspotenziale ist die Nutzung der Technologien in verschiedenen Produktionsstrukturennoch stark unterschiedlich. Lediglich PPS- sowie CAM-Konzepte werden von mehr als der HĂ€lfte der Betriebe eingesetzt. Es stellt sich daher die Frage, welche Potenziale durch die geringe Verbreitung etwa von Industrierobotern (etwa ein Drittel der Betriebe) gegebenenfalls verschenkt werden und fĂŒr welche Einsatz- und Produktionsbedingungen vor diesem Hintergrund zukĂŒnftig noch geeignete Lösungen zu konzipieren sind. --
